What You'll Do

As a Case Manager, you will work directly with clients to assess their needs, coordinate services, and help guide them through treatment and into the community supports that will sustain their recovery. You will collaborate closely with counseling staff, external providers, and community partners as part of a multidisciplinary team dedicated to positive client outcomes.

Responsibilities

include:

Interview clients, complete intake information, review records, and confer with other professionals to evaluate client needs and circumstances

Use motivational interviewing to coach and empower clients toward positive outcomes

Refer clients to other support services as needed, including medical evaluation and treatment, social services, and employment services

Use professional judgment, in conjunction with assigned providers, to plan, develop, and implement effective intervention plans that support a continuum of care and the most positive clinical outcomes

Assist clients with treatment plan objectives through exploration of problems, examination of attitudes and feelings, consideration of alternative solutions, and decision making

Coordinate case management needs, bringing together services, agencies, resources, and clients to assist with discharge planning and referrals

Provide crisis intervention services in response to clients experiencing acute emotional or physical distress

Provide education and information to individual clients and groups on alcohol and drug use disorders, available services, and community resources including additional treatment options and 12-step and other community-based programs

Meet with counseling staff and other relevant stakeholders to report on client progress and discuss and modify established treatment plans as needed

Participate in the orientation of new employees and the transfer of knowledge essential to the performance of duties

What You'll Need

  • Associate's degree preferred; or equivalent knowledge and experience in substance abuse and community support services
  • Valid Driver's License with the ability to drive a company vehicle required
  • Strong communication, organizational, and problem-solving skills
  • Experience with or knowledge of motivational interviewing, crisis intervention, and community resource coordination preferred
  • Commitment to trauma-informed care, recovery-oriented practice, and SCADD's values of diversity, equity, and inclusion
  • Ability to work both independently and collaboratively within a multidisciplinary team
  • Friendly, patient, and empathetic approach when working with clients and external partners

Requirements

Candidates should possess an associate's degree or equivalent experience in substance abuse and community support services. A valid driver's license is required, along with strong communication and problem-solving skills.
