Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and ensuring comfort and dignity.
- Monitor patient health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare teams.
- Support medication administration following proper procedures while maintaining accurate medical documentation and adhering to HIPAA regulations.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe, sanitary environment for clients.
- Assist with specialized care needs such as catheter care, dementia and Alzheimer's care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities.
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions accurately and efficiently.
- Provide emotional support and companionship to foster a positive environment that promotes mental well-being and engagement.
- Follow all safety protocols including CPR certification requirements and proper handling of medical equipment like Hoyer lifts or mobility aids.
Experience
- Prior experience in home health care or long-term care settings is highly preferred.
- Demonstrated ability to work with diverse populations including seniors, individuals with disabilities, or those requiring memory or dementia care.
- Familiarity with medical terminology, medication administration procedures, and documentation standards such as HIPAA compliance.
- Experience using EMR/EHR systems for accurate recordkeeping is advantageous.
- Strong communication skills for collaborating effectively with healthcare teams and family members.
- Certification in CPR is required; additional certifications such as hospice care training or specialized dementia training are a plus.
- Previous work in assisted living facilities or nursing homes provides valuable insight into patient-centered care approaches.
