Key Responsibilities
- Provide comprehensive palliative care in hospital and post-acute environments
- Perform patient assessments, develop individualized care plans, and manage ongoing treatment
- Educate and support patients and families regarding goals of care and treatment options
- Collaborate with physicians, facility staff, and interdisciplinary teams to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
- Active, unrestricted New Jersey NP or PA license (required)
- Experience in palliative care, geriatrics, hospice, chronic disease or related fields preferred
- Strong clinical judgment, communication, and interpersonal skills
- Commitment to compassionate, patient-centered care
