This is an In-person position serving patients in Essex, Hudson, Union, Middlesex, Somerset, Monmouth, and Ocean counties.
Responsibilities:
- Deliver comprehensive palliative care in hospital and post-acute care environments
- Conduct patient assessments, develop care plans, and manage ongoing treatment
- Provide education and support to patients and families regarding goals of care
- Collaborate with physicians, facility staff, and community partners to ensure continuity of care
- Document clinical encounters in compliance with regulatory and organizational standards
- Active and unrestricted New Jersey Nurse Practitioner (NP) or Physician Assistant (PA) license (required)
- Active RN required (APN only)
- Valid DEA required
- Valid CDS required
- Current BLS and ACLS required
- Experience in palliative care, geriatrics, internal medicine, or a related field preferred
- Strong communication, interpersonal, and critical thinking skills
- Commitment to patient-centered, compassionate care
- Competitive pay with bonus potential
- Candidates with 10–15 years of dedicated palliative care experience may qualify for compensation at the higher end of the pay range
- Team Lead roles are available for highly experienced APPs, offering enhanced responsibilities and compensation
