MDS Nurse

Full-Time | Nightingale Nursing & Rehabilitation Center | Erie, PA

Nightingale Nursing & Rehabilitation Center is seeking an experienced and detail-oriented MDS Nurse to join our team in Erie, Pennsylvania. This position plays a vital role in ensuring accurate resident assessments, care planning, reimbursement optimization, and regulatory compliance within our skilled nursing facility.

This is an excellent opportunity for an RN or LPN with MDS experience who is passionate about quality resident care and clinical excellence.

• Complete and coordinate MDS assessments in accordance with federal and state regulations

• Ensure accurate and timely completion of MDS, Care Area Assessments (CAAs), and care plans

• Collaborate with interdisciplinary team members to develop and update individualized care plans

• Review clinical documentation to ensure accuracy and support appropriate reimbursement

• Monitor Medicare, Medicaid, and PPS guidelines related to reimbursement and compliance

• Participate in Quality Assurance and Performance Improvement (QAPI) activities

• Assist with audits, surveys, and regulatory compliance initiatives

• Communicate effectively with residents, families, physicians, and facility staff

• Current RN or LPN license in Pennsylvania

• Previous skilled nursing facility (SNF) or long-term care experience required

• MDS experience strongly preferred

• Knowledge of RAI process, Medicare regulations, and reimbursement systems

• Strong organizational and analytical skills

• Excellent attention to detail and ability to meet deadlines

• Ability to work collaboratively within a multidisciplinary team
