What are the responsibilities and job description for the Case Coordinator position at Niagara Falls Memorial Medical Center?
Niagara Falls Memorial is in growth mode and currently looking for a Case Coordinator to join our team!
The Case Coordinator will be connecting emergency department patients to primary care appointments within seven days. As the case coordinator, you’ll be working on an innovative project that emphasizes the importance of the overall health and wellness and linkages to primary care. This role requires a case coordinator to help the patients overcome any issues that may create an obstacle to attending a primary care appointment such as arranging medical transportation, referring patient to case management services and providing patient education on the importance of follow up care in the community. The case coordinator will be responsible for ensuring a 7-day primary care appointment, making reminder phone calls, communicating with primary care staff and tracking the results of interventions.
Requirements:
- Bachelor’s Degree in Social Work, Psychology or a related field
- 1-year experience in healthcare preferred
- Experience with Excel and tracking data
