The Niagara County Department of Mental Health & Substance Abuse Services is seeking an experienced Case Manager for our Assisted Outpatient Treatment program!! We are seeking a Mental Health Case Manager (MHCM) for our Assisted Outpatient Treatment (AOT) Program. The AOT program serves individuals who experience serious mental illness who struggle with maintaining stability in the community and therefore experience significant risk. Services are provided via voluntary agreements or court orders. The Mental Health Case Manager reviews referrals to the AOT program, assists in gathering documents and reports to evaluate referrals, provides input into the development of assessment and AOT program/service recommendations. The MHCM actively monitors and documents compliance with services outlined in AOT Voluntary Service Contracts or Court Orders, coordinates among service providers, and addresses concerns and barriers to care.
Additionally, the MHCM coordinates with the AOT Psychiatrist and assigned legal counsel and prepares Voluntary Service Agreements and Court documents. MHCM attends case conferences, court hearings, and engages clients in the community (home/hospital/outpatient provider offices) as needed. The AOT program works in conjunction with Niagara County Crisis Services and the Mobile Crisis Team serving those at risk of hospitalization with the goal of hospital diversion where possible.
Typical work hours are Monday – Friday 8:30 – 4:30, 35 hour/week. The Mental Health Case Manager also participates as a part of our after-hours mobile crisis team, with occasional after hours paid on-call - 2 - 3 days per month. Candidates must have a Bachelor's Degree and two (2) years qualified relevant experience or an Associate's degree and four (4) years qualified relevant experience. Qualified experience includes: case management or crisis services experience, working in a mental health, alcohol/drug setting, or related setting where responsibilities directly involve working with mental health and/or addiction related issues or social case work concerns.
