Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient observations, vital signs, and overall health status to detect changes or concerns promptly
- Provide reminders for medications following proper protocols and document medical records accurately using Electronic Health Record (EHR) systems
- Support clients with specialized care needs such as Alzheimer's, dementia, memory care, hospice care, and catheter care
- Operate medical equipment like Hoyer lifts and perform safe transfers to prevent injury
- Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe living environment
- Document all patient interactions, medical documentation, and care activities in compliance with HIPAA regulations
Skills
- Strong knowledge of medical terminology and caregiving best practices in home & community care settings
- Experience working with individuals with developmental disabilities or disabilities requiring long-term care
- Proficiency in EMR (Electronic Medical Records) and EHR systems for accurate documentation
- Ability to perform patient observation, vital signs measurement, medication administration, and dementia or Alzheimer's care
- CPR certification and familiarity with hospice or palliative care procedures
- Skilled in assisting with ADLs using equipment like Hoyer lifts and managing catheter or other specialized medical needs
- Compassionate communication skills to provide emotional support and build trusting relationships with clients
