Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly to healthcare providers
- Support medication administration following proper protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) systems
- Provide companionship and emotional support to clients living with dementia or Alzheimer's disease, ensuring a safe and comforting environment
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a clean and safe living space
- Assist with medical procedures like catheter care and hospice-related tasks under supervision or guidance from nursing staff
- Maintain strict compliance with HIPAA regulations to protect patient privacy and confidentiality at all times
Qualifications
- Proven experience in home health care, assisted living, nursing homes or long-term care environments
- Strong knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation
- Certification in CPR is required; additional certifications such as medication administration or hospice care are a plus
- Experience working with individuals with developmental disabilities or dementia enhances your ability to provide specialized care
- Ability to perform patient observation accurately and respond effectively to changing needs or emergencies
- Valid driver’s license and reliable transportation preferred for live-in or community-based assignments
- Compassionate attitude combined with excellent communication skills to build trust with clients and families
