What You’ll Do
- Coordinate and oversee restorative nursing programs for assigned residents
- Work closely with the Director of Nursing, therapy teams, and nursing staff to support restorative care goals
- Participate in care planning and the MDS/RAI process related to restorative services
- Evaluate residents’ functional abilities and identify opportunities to improve mobility and independence
- Initiate therapy referrals and serve as the liaison between therapy services and nursing staff
- Monitor resident progress and adjust restorative programs as needed
- Educate residents, families, and staff on restorative care practices
- Ensure restorative care policies and procedures are followed
- Active Registered Nurse (RN) license in good standing
- Experience working with elderly or disabled populations, preferably in long-term care
- Understanding of restorative nursing programs and care planning
- Strong leadership and communication skills
