The Restorative Nurse is responsible for coordinating and overseeing restorative nursing programs for residents throughout the facility. This individual will help bridge communication between nursing and therapy departments while ensuring restorative interventions are properly carried out, documented, and aligned with each resident’s care plan and goals. The ideal candidate is organized, compassionate, clinically knowledgeable, and passionate about improving resident outcomes.
Key Responsibilities
- Coordinate and oversee the facility’s restorative nursing program
- Collaborate with the Director of Nursing, therapy team, physicians, and interdisciplinary team members regarding restorative care needs
- Participate in the MDS/RAI process and care plan development related to restorative nursing programs
- Identify residents appropriate for restorative services and initiate therapy referrals/evaluations as needed
- Monitor resident progress and update restorative programs based on changing needs and goals
- Ensure restorative nursing documentation is completed accurately and timely
- Educate residents, families, and staff on restorative nursing interventions and goals
- Supervise restorative nursing assistants and provide direction to nursing staff related to restorative care
- Promote resident independence, mobility, strength, and overall quality of life
- Ensure compliance with all federal, state, and facility regulations and policies
- Current Licensed Practical Nurse (LPN) license in good standing
- Long-term care or skilled nursing experience preferred
- Knowledge of restorative nursing practices, care planning, and documentation
- Strong communication, leadership, and organizational skills
- Ability to work collaboratively with therapy and nursing departments
