✅ Key Responsibilities
- Assess residents and develop, implement, and evaluate individualized care plans
- Administer medications, treatments, and physician-ordered interventions
- Monitor residents for changes in condition and communicate findings appropriately
- Supervise and support LPNs, CNAs, and other nursing staff
- Collaborate with rehabilitation, therapy, dietary, and interdisciplinary teams to support resident recovery goals
- Maintain accurate, timely clinical documentation in accordance with facility policies and state and federal regulations
- Educate residents and families regarding care plans and health needs
- Promote a safe environment while following infection prevention and safety protocols
- Current and valid Licensed Practical Nurse ( LPN ) license
- Strong clinical assessment and critical thinking skills
- Excellent communication and leadership abilities
- Ability to work effectively in a collaborative, fast-paced environment
- Compassionate, resident-focused approach to care
- Long-term care or rehabilitation experience preferred, but not required
- BLS/CPR certification required
