Job Summary
The Case Manager (RN, LPN, or Social Worker) supports patients across the Hoover and greater Birmingham, Alabama area, coordinating care transitions, discharge planning, and access to post-acute services—similar to a hospital case management role, but with a more predictable schedule and manageable caseload.
Responsibilities
- Coordinate care transitions and discharge planning
- Collaborate with nursing, therapy, physicians, and families
- Arrange home health, DME, rehab, and placement services
- Connect patients with Birmingham-area community resources
- Manage documentation and ensure timely, accurate records
- Active Alabama license (one of the following required):
- Registered Nurse (RN)
- Licensed Practical Nurse (LPN)
- LMSW or LCSW (Social Worker)
- Experience in case management, discharge planning, or care coordination preferred
- Strong communication, organization, and problem-solving skills
- Competitive pay $26.50 – $32.40/hr (based on experience)
