Case Manager-Post Permanency Support Program St. Albans

Join our compassionate team at the NFI St. Albans Post Permanency Support Program specializing in working with families created through adoption and/or guardianship. We are dedicated to providing a supportive environment for both our clients and staff, prioritizing therapeutic interventions and team camaraderie. 

Responsibilities:

•  Provide information on the unique aspects, issues and challenges of adoption, kinship, and guardianship and support families through their experiences.
•  Offer in home parent education and support around early trauma-based behaviors and collaborate with parents and teams to develop trauma informed strategies to address the behavior. 
•  Offer consultation and advocacy to the family’s treatment team including schools, treatment providers, and other support team members, particularly around impact of early childhood trauma and adoption.
•  Support parents at team meetings
•  Facilitate a monthly support group to provide connection and information for families. 
•  Provide information about and make referrals to community and regional resources. 
•  Schedule monthly meetings with parents
•  Contact collateral team members monthly
•  Support parents in developing, working on and documenting goals with regular updates
•  Maintain a database with notes of client and collateral contacts
•  Record and send monthly billing and other record keeping documents to appropriate parties
•  Enroll clients and obtain required documentation for enrollment

Requirements:

Education:  Bachelor’s Degree

Good computer skills

Strong communication skills.

Good organizational skills.

Ability to work independently.

Pay/Benefits:

• Full-time
• Pay range: 50,000 – 52,000
