Description

Nexus-Woodbourne is seeking a licensed social worker or counselor that is well-versed with Trauma-Informed practices and clinical diagnosis treatment processes to provide mental and behavioral health services to male identifying youth, ages 12 - 18. To better serve our youth & teen clients, our Mental Health Therapist is required to work ON-SITE. This is NOT a HYBRID or REMOTE role.

Salary Range - $70,000 - $77,000

Position Summary: Provide overall management of youth’s treatment through active engagement, clinical assessment, treatment, and discharge planning. Therapist will provide clinical insight and work from a trauma informed lens to enhance the milieu and programming. Engage with families through family therapy and treatment planning. Collaborate with the community and providers related to the youth’s progress and discharge planning.

Primary Responsibilities

  • Trauma Informed Care
  • In coordination with the team, creates and drives a trauma-informed environment; establishing and maintaining a positive treatment culture and making necessary changes to ensure success.
  • Collaboration with youth and family to lead and develop to the Nexus Individual and Family Guidance Plans; ensuring staff understand and utilize them in adherence to policy.
  • Engage in and have active involvement of the Family Collaborative Intake meetings.
  • Complete Child and Adolescent Needs and Strengths functional assessment in accordance to the TCOM (collaborative) principals.
  • Through the collaborative effort with family, youth, and providers, develop and monitor the individual treatment plans.
  • Provide individualized, youth-focused, strength and evidenced-based family and individual therapy models to youth and families. Examples: Trauma Focused Cognitive Behavior Therapy and Eye Movement Desensitization and Reprocessing.
  • Attendance and participation in mandatory clinical supervision and team case consultation meetings in accordance with program expectations.
  • Milieu Coordination
  • Develop and facilitate milieu and treatment groups that are trauma informed, educational, and treatment focused.
  • Participate in all youth related treatment team meetings, using family centered language and youth driven principals.
  • Serve as a clinical treatment team leader to YCP's. This includes but is not limited to modeling and coaching staff on rapport building with clients, trauma-informed practices, and de-escalation techniques on the treatment milieu as needed.
  • Be available on-call for consultation/treatment direction and/or decisions when appropriate.
  • Assess readiness for discharge in conjunction with the treatment team. Coordinate and recommend therapeutic services for success after discharge.
  • Community Care
  • Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition.
  • Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process.
  • Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional manner according to policy and regulated statutes.
  • Educate and facilitate trainings for staff on trauma informed practices, clinical diagnosis treatment processes, and other clinically relevant training material.
  • Participate in team meetings, program clinical team meetings, therapy clinical supervision sessions, and in-service as required.
  • Ensure confidentiality regarding mental health, medical issues, background treatment and comply with all HIPAA related requirements.
  • Remains current on all company policies, assigned trainings and/or certification requirements. h. All other tasks and duties as assigned.

Mental Health Therapist Leveling

Therapist I: Unlicensed, Intermediary, Practice License level. Minimum of a master’s degree from an accredited college or university in one of the behavioral sciences, human services or related fields (i.e. Social Work, Psychology, Counseling, or Marriage and Family Therapy). If state requires, must be on track for full clinical licensure for a specific, related discipline and/or currently hold a provisional/intermediary license for a specific track.

Maryland: LCPC, LCSW, LCSW-C

Requirements

Required Education and Licensure:

  • Master’s degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields
  • If currently unlicensed and clinical licensure is required in your state: o Must be on track to complete licensure for a specific, related discipline
  • And/or currently holds a provisional/intermediary license for a specific track (i.e. LGSW, LMSW, LAMFT or LPC)
  • In Illinois: Must have or be willing to obtain an LASOP license within 3 months of hire.
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements

Preferred Education And Experience

  • Fully licensed in appropriate state as an LCSW, LICSW, LP, LMFT, LPCC, or LCPC, LCSW-C
  • In Illinois: LSOTP licensure
  • Related residential or human services experience working with youth and families
