Position Summary
The Social Services Director plans, organizes, and directs the social services program to meet residents' emotional, social, and psychosocial needs and to support successful admissions, transitions, and discharges.
Responsibilities
Oversee social services for residents, including assessments, care planning, and ongoing support for emotional and social needs.
Coordinate discharge planning, including resources, referrals, and family communication.
Advocate for resident rights and preferences and participate in care conferences.
Maintain required documentation and reports in accordance with regulations and facility policy.
Collaborate with nursing, therapy, activities, and other departments to support holistic resident care.
Supervise social services staff as applicable and provide leadership and guidance.
Qualifications
Bachelor’s degree in Social Work required.
At least 3 years of long-term care/skilled nursing experience required.
At least 3 years of MDS or related case-management/care-planning experience required.
Strong communication, advocacy, and problem-solving skills.
Ability to work effectively with residents, families, and an interdisciplinary team.
