Full Time Position Available Monday-Friday 8:00am-4:30pm

Successful Candidates:

• High Schoolgraduate or GED required
• KansasCertified Nurse Aide Certification and Kansas Home Health Aide Certification
  
• CurrentBLS and driver's license
  
• Becustomer-oriented and possess the ability to work independently with minimal supervision

Essential Functions:

• Perform basis assessment of patients and provide services that are included in the patient's care plan
• Report measurable outcomes as requested and communicate and changes in the patient's medical, nursing, rehabilitative, and social needs to the hospice nurse
• Provide support and education to the patient/family and/or caregiver about the patient's disease process
