Overview
We are seeking a dedicated and compassionate Case Manager to join our Housing Supports Program team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs and facilitating access to necessary resources. This is a direct service position interfacing with persons who are insured by Medicaid Managed Care Organizations (MCO) and facing homelessness and/or housing challenges. Case managers will need to interact with various individuals, departments and benefits organizations to advocate on behalf of their clients. Case managers will work with people in the community or at a NewBridge site to complete applications, identify resources, help with obtaining and maintaining housing, and teach relevant skills and strategies to empower clients on their journey toward stability and self-sufficiency.

Responsibilities

  • Conduct comprehensive assessments of clients’ needs and circumstances to determine eligibility for housing supports program, including identifying clinical and social risk factors.
  • Develop, implement, monitor, and renew individualized housing support plans tailored to each client.
  • Facilitate access to housing resources, including navigating LIHTC and Section 8 applications.
  • Provide practical assistance with housing search, applications, landlord/tenant relations, review of living environment needs, move coordination, and obtaining resources to obtain and retain housing.
  • Advocate with outside programs for services and follow-up to ensure successful linkage with other services/agencies.
  • Provide outreach services through home visits, phone calls, contact with significant others and other relevant persons to meet the client’s needs.
  • Maintain accurate records of client interactions, progress, and outcomes in compliance with organizational policies and regulatory authorities.

Skills & Experience

  • Case Management experience, especially with special populations
  • Experience with Low-Income Housing Tax Credit (LIHTC) programs and Section 8 housing assistance.
  • Ability to travel throughout Northern NJ to reach clients and services, using own vehicle (with mileage reimbursement)
  • Exceptional communication skills, both verbal and written, with the ability to build rapport with diverse populations.
  • Lived experience of unstable housing or homelessness and/or as a NJ FamilyCare participant will be favorably considered

This is a hybrid opportunity with up to 7 hours/week available as work from home, with the remainder being working onsite or in the field.

Pay: $19.98 - $22.06 per hour
