What are the responsibilities and job description for the Housing Supports Case Manager position at NewBridge Services, Inc.?
Outreach case manager serving people who are Medicaid MCO recipients, homeless, or do not have stable housing. The clients will need assistance with connecting to supportive services and developing life skills to ensure that they obtain and remain housed. Staff acts as a liaison with other social service providers and external institutions in order to connect clients with benefits and entitlements. Drivers license and HS Diploma required.
