NYTPS is hiring a Bilingual (Spanish) Early Intervention Ongoing Service Coordinator (OSC) for the Nassau and Suffolk County Early Intervention programs. We are looking for energetic, motivated, and organized candidates who are committed to helping families in the community. Responsibilities:
- Assist families in navigating and understanding the Early Intervention process.
- Coordinate Early Intervention evaluations and services for children, ensuring alignment with family and provider availability.
- Contribute to the development, implementation, and ongoing monitoring of the Individualized Family Service Plan (IFSP).
- Maintain consistent communication with families, service providers, and NYTPS staff.
- Oversee service delivery, ensuring timely quarterly progress reports and conducting semi-annual IFSP reviews or amendments.
- Accurately document all interactions with families and Early Intervention providers, as well as any relevant information related to the child’s case.
Qualifications: Candidates must be fluent in Spanish AND meet one of the following education and experience criteria:
- Two years of experience in service coordination.
- One year of service coordination experience and one year of experience working in a service setting with infants and toddlers with developmental delays or disabilities.
- One year of service coordination experience and an associate degree in health or human services.
- A bachelor’s degree in health or human services.
