Registered Nurse Surveyor – Syracuse, NY (2 Positions)
The New York State Department of Health is seeking Registered Nurses to join the Central NY Regional Office in Syracuse to support oversight of Adult Care Facilities and Long-Term Care programs.
Position Overview:
The RN Surveyor will work with an interdisciplinary team to conduct onsite inspections and complaint investigations at Adult Care Facilities and nursing homes across a 14-county Central New York region. Responsibilities include evaluating facility compliance with state and federal regulations, reviewing complaints and incident reports, conducting interviews and observations, and preparing survey reports and documentation.
Minimum Qualifications:
- Current Registered Nurse license in New York State
- Strong written and verbal communication skills
- Proficiency with Microsoft Office (Word, Excel)
- Ability to manage multiple priorities and work collaboratively on a team
Preferred Qualifications:
- Experience in long-term care, assisted living, or nursing facility oversight
- Knowledge of regulatory compliance and healthcare investigations
- Familiarity with ASPEN databases, SharePoint, and Electronic Plan of Correction systems
Key Responsibilities:
- Conduct onsite surveys and complaint investigations at Adult Care Facilities and nursing homes
- Review facility records, incident reports, and policies for regulatory compliance
- Interview residents, staff, and relevant witnesses
- Prepare survey reports and documentation using state systems
- Participate in enforcement actions and regulatory meetings when required

Travel: Field inspections within a 14-county region
