What are the responsibilities and job description for the Mandarin and Cantonese Speaking Home Care Coordinator - Full time (TEMP) position at New York Foundation for Senior Citizens Home Attendant Services, Inc.?
Bilingual: English/Mandarin/Cantonese
Supervises and insures home care worker accountability, in cooperation with the RNs.
Insures that all Home Attendants are oriented to their clients’ respective needs and plansof care. Calls the client to verify the arrival of the Home Attendant and completes and places a progress note in the computerized Client Profile.
Case management and follows up on home care worker and client need.
Insures continuous client coverage in accordance with authorized hours.
Inputs all changes in service hour authorizations into computer system immediately upon receipt of such authorizations.
Insures that client records are up-to-date and complete and thatthey are continually maintained in all areas of compliance contract.
Logs time sheets, processes timesheet for payroll and assists in resolving payroll problems.
Liaison with HRA, MCOs, MLTCs to notify them of any changes in client needs.
Prints EPaces Medicaid eligibility for MCO,MLTC and HRA clients on the 1st and 15th of each month.
Rotates on call with other administrative staff, both weekly and weekends.
Answers calls from home attendants relating to payroll and timesheet.
