Position Summary
We are seeking a Case Manager (Qualified Mental Health Specialist) to join our care team. In this role, you will provide direct support, care coordination, and advocacy for clients with mental health needs. You’ll work closely with our clinical team to link clients to resources, create care plans, and help them reach their goals.
What You’ll Do
- Provide case management and care coordination for clients with behavioral health needs.
- Develop, implement, and monitor care plans with clients and their families.
- Connect clients with community resources, wellness activities, and support services.
- Collaborate with the clinical team to ensure whole-person care.
- Maintain documentation and uphold compliance standards.
Qualifications
- High school diploma/GED required; associate’s or bachelor’s in social work, psychology, or related field preferred.
- 2 years of care coordination or case management experience (if degree is not in a related field).
- Valid Ohio driver’s license and insured vehicle.
- Strong communication, decision-making, and organizational skills.
- Ability to work effectively with diverse populations.
- Must be able to service clients in Cuyahoga County
