POSITION SUMMARY
The Case Manager is primarily responsible for assisting in patient care to promote effective education, self-management support and timely healthcare delivery to achieve optimal quality and financial outcomes. This individual is a key member of the Clinical Team and services as an advocate for patient mental health needs – coordinating care and discharge planning to minimize the fragmentation of healthcare delivery systems and thereby enhancing quality, cost-effective outcomes.
Essential Duties
· Specialties include the treatment of eating disorders, mental health and substance abuse.
· As a Case Manager you will facilitate the child and family team process and serve as lead for Highly Complex cases
· Case Managers will meet with patients and families when needed to coordinate behavioral health services, medical care, education, placement, and other needs during treatment and at the time of discharge
· You will have targeted case management clients who typically need medication and one other short-term interventions such as individual and/or family therapy. To include step down to RTC, PHP, IOP and other aftercare options.
· As a Case Manager, you will work with other dedicated professionals who share your passion for helping individuals in need
· As a case manager, you will run a daily. Non-clinical group with identified patients.
Qualification Requirements
· 3 years of related behavioral health experience with High School Diploma/GED OR 2 years of related behavioral health experience with bachelor’s degree or higher.
· Customer service experience required
· Basic computer skills required - Knowledge of Outlook email, Word, Excel, etc.
· Must be 21 years of age or older
· Valid NM Driver's License
· Must be able to receive NMDOH Fingerprint Clearance
· Must be able to pass background check and drug screening
· Must be able to complete and provide proof of current influenza vaccination or be willing to be vaccinated or able to sign a declination from.
Language Skills
· Ability to read, analyze, and interpret human service documentation, professional journals, technical procedures, governmental regulations, and electronic medical record documentation.
· Ability to write reports and communicate with patient family and external case managers during discharge planning meetings.
· Ability to effectively present information and respond to questions from groups of managers, clients, customers, and insurance are coordinators.
· Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions.
Computer Skills
To perform this job successfully, an individual should have knowledge of Electronic Health Records and MS Office software and use of the internet. Must be able to see and read a computer monitor or notebook screen.
