Duties
- Conduct thorough assessments to identify clients’ needs related to behavioral health, addiction, child welfare, and social services.
- Develop individualized care plans and coordinate appropriate resources such as Section 8 housing, Medicare services, or workforce development programs.
- Provide crisis intervention and crisis management to stabilize clients during emergencies or periods of heightened stress.
- Utilize motivational interviewing techniques to encourage positive behavioral change and engagement with services.
- Support clients through supervision and mentoring, fostering independence and self-sufficiency.
- Collaborate with healthcare providers, social service agencies, and community organizations to ensure seamless service delivery.
- Monitor client progress regularly, adjusting care plans as needed to meet evolving needs.
- Maintain detailed documentation of interactions, assessments, and service plans in compliance with organizational policies.
Requirements
- Proven experience in social work, behavioral health, or related fields such as addiction counseling or child welfare.
- Strong skills in crisis intervention, crisis management, and motivational interviewing.
- Supervising experience preferred, especially in mentoring or childcare settings.
- Knowledge of programs like Section 8 housing, Medicare, and workforce development initiatives.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Ability to handle sensitive situations with professionalism and empathy.
- Relevant certifications or licenses in social work or counseling are a plus.
