Wellness Coordinator (part-time) - Community Living
New Horizons is looking for a Wellness Coordinator employee (Nurse) to assist adults with intellectual and developmental disabilities to reach their goals, grow, develop skills and live as independently as possible in the community. Additionally, a Wellness Coordinator is responsible for the development, maintenance and routine monitoring of the individual waiver participant's wellness plan and the medical services required to manage the health care needs of the individual.
A successful Wellness Coordinator employee will have excellent communication and interpersonal skills to foster a positive relationship with the individual(s) they support. The Wellness Coordinator will need to utilize critical thinking, time management, and organizational skills while supporting the individual(s) they support.

Job Qualifications
- Nursing degree from an accredited school is required
- Have and maintain a valid nursing license (RN or LPN)
- Valid Driver's License
- Complete required training including First Aid/CPR certification
- Drug Test required
- Background report and driving record required
- TB test required
