What are the responsibilities and job description for the Case Manager (APEX) position at New Horizons Behavioral Health?
Under immediate to general supervision, the candidate will provide case management to youth, ages 5 to 17, on the school's campus. Other duties include:
- Assisting with linking youth and families to community resources
- Managing referrals
- Conducting skill-building groups
- Serving as liaison between New Horizons Behavioral Health and the school(s).
- Participating in recruitment of individuals to the program by attending PTA and Open House meetings at schools
- Completing clinical documentation.
- Other duties as assigned.
Program Name: APEX - Harris County
Location Served: Harris County School District
Location Position Will Be Housed: Harris County Carver Middle School
Works with the following population: Male and female youth, ages twelve (12) to fifteen (15).
Minimum Requirements: High school diploma / GED with three (3) to five (5) years of experience in this field -OR- the completion of a Bachelor's degree in human services / social science (Social Work, Psychology, Sociology, Counseling, or a related discipline) and one (1) year of full-time experience working with individuals with mental health disorders. The Bachelor's degree must be in the above named fields to be MHC eligible (Mental Health Clinician). Must have a driver's license, an insured vehicle and a clean driving record, as this position is required to transport.
Preferred Qualifications: A Bachelor's Degree and experience in working with individuals with mental disorders.
Days & Time: This position will work 40 hours a week. Monday through Friday, 8am to 5pm. Most programs work from 8am to 5pm, with a one-hour lunchbreak.
Pay: $31,000 per year
