Job Overview

We are seeking a dynamic and compassionate Resident Care Manager to lead and coordinate resident care services within our long-term care facility. In this vital role, you will oversee daily operations, ensure compliance with healthcare regulations, and foster a nurturing environment for residents. Your leadership will drive quality care delivery, staff supervision, and operational excellence, all while maintaining a focus on resident safety, dignity, and well-being. Duties

  • Supervise and coordinate nursing staff to deliver exceptional patient care aligned with facility policies and healthcare standards.
  • Oversee resident assessments, care planning, and documentation utilizing EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.
  • Ensure compliance with HIPAA regulations, state healthcare laws, long-term care regulations, and NCQA standards.
  • Conduct regular staff training on medical documentation, infection control protocols, medical terminology, and safety procedures such as sterile processing and aseptic technique.
  • Collaborate with interdisciplinary teams to develop discharge planning strategies, behavioral health plans, dementia care programs, and family communication.
  • Monitor vital signs, perform physical examinations when necessary, and oversee patient assessment processes including diagnostic evaluation and laboratory experience.

Skills

  • Extensive experience working with individuals with disabilities, geriatrics, dementia care, behavioral health needs, or developmental disabilities.
  • Proficiency in EMR/EHR systems such as Cerner or Athenahealth; familiarity with ICD-9/10 coding; CPT coding knowledge preferred.
  • Strong leadership skills with supervising experience in nursing management or nurse leadership roles within long-term or acute care settings.
  • Knowledge of hospital medicine environments including ICU or Level I/II trauma center experience; familiarity with critical & intensive care procedures such as ventilator management or airway management is a plus.
  • Ability to perform complex patient assessments involving physiology knowledge and diagnostic evaluation techniques like sonography or spinal tap procedures.
  • Skilled in managing care teams effectively through managerial communication and ensuring nursing policy compliance while adhering to long-term care regulations.
  • Competence in managing medical records accurately while ensuring HIPAA compliance; experience with medical imaging or laboratory processing is advantageous.
  • Demonstrated ability to handle emergency situations such as acute pain management or infection control incidents confidently and efficiently.
  • Provide leadership support by assisting with staffing needs, covering open shifts as needed, and participating in the on-call rotation to ensure continuity of resident care and timely response to staff and resident concerns.
