New Foundations LLC is an operator of Mental Health Residential Treatment homes providing specialized services to 5 individuals living in a comfortable, home-like setting. We are looking for an outgoing, motivated clinician to join our dedicated team! In this role, you will provide vital mental health counseling and treatment services to better our program and directly serve residents living with severe and persistent mental illness (SPMI) as well as other co-occurring diagnoses.

Our non-secure, community-integrated facility operates under OAR 309-035 guidelines and serves a diverse adult population, including individuals navigating forensic Aid and Assist (.370) court mandates and Civil Commitment orders.

Principal Duties & Responsibilities

  • Assessments: Conduct clinical screenings and comprehensive mental health assessments upon entry. Evaluate clients’ mental condition and needs based on chart reviews, clinical interviews, observation, and testing.
  • Treatment Planning: Develop, implement, and document person-centered individual treatment plans. Monitor and evaluate progress monthly, modifying plans as needed.
  • Therapy: Conduct regular individual and group counseling sessions utilizing evidence-based modalities (CBT, DBT, etc.).
  • Habilitative Training: Deliver structured skill-building targeting the 14 core OHA residential domains, including Activities of Daily Living (ADLs/IADLs), emotional regulation, financial literacy, communication, social engagement, and community navigation.
  • Staff Support: Provide clinical training, guidance, and daily support to direct care staff.
  • Milieu Management: Maintain a structured, positive therapeutic environment and participate in monthly administrative meetings.
  • Crisis Intervention: Assist with proactive behavioral de-escalation and crisis intervention when necessary.
  • Compliance: Maintain accurate client records in strict accordance with professional ethics, certification bodies, and OAR 309-035 standards.

Employment Qualifications

  • Education: Master’s degree in Counseling, Psychology, or a related behavioral science field (Required).
  • Licensure: Current Oregon state license as a medical professional or equivalent professional license (Must maintain current professional license requirements).
  • Credential: Active QMHP certification issued by the Mental Health & Addiction Certification Board of Oregon (MHACBO), or registered as a QMHP-Registrant (QMHP-R).
  • Background Check: Must clear a mandatory criminal history background check managed by the Oregon Department of Human Services (DHS) / Oregon Health Authority (OHA).

Required Experience

Candidates must demonstrate a documented ability to:

  • Conduct clinical assessments, identify precipitating events, and gather comprehensive mental, physical, and substance use histories.
  • Conduct formal mental status examinations and complete a DSM diagnosis.
  • Write and supervise the implementation of complex service/treatment plans.
  • Provide individual and group therapy within the scope of their training.

Pay: From $32.00 per hour
