Overview
We are seeking a compassionate and highly skilled Hospice Registered Nurse to join our dedicated healthcare team. The ideal candidate will possess extensive clinical experience, including familiarity with EMR/EHR systems such as Epic, Cerner, and Wellsky, and have a strong background in palliative and end-of-life care. This role involves providing holistic patient care in a hospice setting, ensuring comfort, dignity, and quality of life for patients with life-limiting illnesses. Experience in diverse clinical environments such as critical care, hospital medicine, long-term care, and home care is highly valued. The Hospice RN will collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with HIPAA regulations and NCQA standards.
Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Manage complex symptom control including acute pain management, dyspnea relief, and comfort measures tailored to hospice patients.
- Coordinate discharge planning and facilitate transitions between inpatient, outpatient, home care, or assisted living settings.
- Document all patient interactions accurately within electronic health records (EHR systems) like Epic or Cerner while maintaining compliance with HIPAA standards.
- Provide education on disease processes such as dementia or Alzheimer's care, medication administration, and advanced directives to patients and families.
- Collaborate with interdisciplinary teams including physicians specializing in hospice & palliative medicine, behavioral health specialists, social workers, and case managers to optimize patient outcomes.
- Participate in ongoing professional development related to hospice nursing practices, infection control protocols, trauma medicine (Level I & II trauma center experience), and specialized areas like pediatric or neonatal ventilator management when applicable.
- Respond promptly to urgent situations requiring airway management or ventilator support; perform emergency procedures such as CPR when necessary.
- Support community outreach efforts by providing health coaching and working with individuals with disabilities or chronic conditions in various settings including home & community environments.
Skills
- Proven proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth; familiarity with CPT coding and ICD coding for accurate documentation.
- Strong knowledge of medical terminology, anatomy & physiology, diagnostic evaluation techniques.
- Ability to perform physical assessments accurately; adept at vital sign monitoring and interpreting laboratory results for clinical decision-making.
- Knowledge of infection control practices; experience working within hospital medicine units such as ICU or trauma centers is advantageous.
- Excellent communication skills for teaching patients/families about disease management; capable of working with individuals across age groups from neonatal to geriatric populations including those with developmental disabilities or dementia.
Pay: $94,000.00 - $101,000.00 per year
