What are the responsibilities and job description for the Care Coordinator position at NEW DIRECTIONS FOR WOMEN INC?
Schedule: 2:30pm - 11:00pm
Key Areas of Responsibility:

Clinical and Groups:
- Responsible for tracking attendance in groups.
- Responsible for charting groups/activities not facilitated by clinical staff
- Responsible for gathering the clients in treatments at the time of group and communicating attendance concerns to ensure clients are meeting their direct service hour requirements through insurance
- Responsible for monitoring client behavior and sharing with appropriate team members when concerns present
- Responsible for supervising chores and conducting room checks.
- Responsible for acting as a model and mentor to the client in treatments.
- Responsible for conducting perimeter checks

Medical
- Responsible for observing medications per the Medication Policies and Procedures
- Responsible for collecting UA drug screens and recording the results in the electronic medical record

Transportation
- Responsible for providing and facilitating the transportation needs for clients in treatment to approved appointments and scheduled activities
- Responsible for providing and facilitating transportation needs for clients for pick up, drop off and/or transition to and from their treatment episode

Other Duties
- Responsible for timely documentation of client treatment episode in the EMR (I.e., UAs, progress notes, shift notes, MAR, vitals, COWS/CIWA, etc.)
- Responsible for providing childcare coverage when childcare staff are on break or unavailable
- Responsible for assisting the chef with kitchen cleanliness, organization, meal prep and service, and food labeling when needed
- Responsible for assisting housekeeping with upkeep and cleanliness of the facilities
- Responsible for completing and filing required forms (I.e., Hourly Checks, Shift Change, Weekly Walkthrough, Pre-Trip Inspection, etc.)
- Responsible for completing property, gown, and room searches as needed to ensure client safety
- Responsible for engaging in and staying up to date with all assigned trainings
- Responsible for maintaining consistent communication via email and other approved platforms
Skills/ Special Requirements:
Must possess current California driver’s license or Identification Card.
Must possess or obtain CPR and First Aid Certifications in allotted timeframe.
Clear Probation Check with Orange County Probation prior to employment starting.
Must have sensitivity to and ability to embrace individuals of different genders, age, sexual orientation, ethnicity, religion, physical abilities and those with diverse cultural backgrounds.
Must possess a current RADT-1 Certification or similar credential or receive one in the allotted timeframe.
Attendance in appropriate support groups is required, as directed by supervisor, in order to gain a better perception of the disease they will be working around or with on a daily basis in order to model 12 step principles. (AA, NA, OA, Al-Anon, etc.)
Possess the ability to make independent decisions when circumstance warrants such action.
Possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
TB test to be conducted yearly or be verified by a physician.
