Position Summary:
The Care Navigator serves as the frontline connection for individuals who contact New Birth Missionary Baptist Church seeking financial assistance, resource support, spiritual care, and emotional guidance. This role is responsible for assessing needs, offering ministerial support, and guiding individuals through the church’s “Care System”, which includes community resources, bereavement care, and counseling services. The Care Navigator ensures that each individual receives compassionate attention, prayer, and strategic guidance while being connected to the appropriate internal or external resources. The omission of specific duties for this position does not exclude them if the work is similar or related. Other duties may be required and assigned.
Key Responsibilities:
Ministerial and Emotional Support
· Serve as the first point of contact for individuals in need of financial or resource assistance.
· Provide spiritual encouragement, prayer, and uplifting conversation to those seeking support.
· Demonstrate Christ-centered care, offering hope and guidance to those in crisis or transition.
Assessment and Referral
· Conduct individualized care assessments to determine the specific needs of each person.
· Develop a Personalized Care Plan for each case, guiding individuals to the appropriate support services.
· Refer individuals to the appropriate department, including:
o Community Resources Department for financial assistance and benevolence support.
o Bereavement Ministry for grief support and funeral assistance.
o Counseling Services for mental health, emotional healing, and spiritual guidance.
Other internal church ministries or external partners that align with the individual's needs.
Case Management & Follow-Up
· Maintain accurate documentation of interactions, capturing the individual’s story, need, and response plan.
· Responsible for gathering confidential information and documentation from clients.
· Track referrals and follow-up to ensure individuals receive the help they need.
· Collaborate with internal teams and external organizations to provide holistic care solutions.
Collaboration & Community Engagement
· Work closely with pastoral staff, Community Resources Department, outreach teams, and community partners to expand support resources.
· Stay informed about available local, state, and national assistance programs to provide comprehensive referrals.
· Participate in church and community outreach efforts, ensuring accessibility to care resources.
Administrative & Training Responsibilities
· Develop and maintain a resource directory of internal and external support services.
· Train and support volunteers or ministry teams assisting with outreach and care initiatives.
· Provide reports and updates to the Director of Ministries and Outreach regarding trends, case outcomes, and areas of need.
Qualifications:

Education & Experience
· Bachelor’s degree in Pastoral Care, Social Work, Psychology, Counseling, or a related field preferred.
· 2 years of experience in pastoral care, social services, case management, or a similar role.
· Experience in crisis intervention, community outreach, or counseling is highly desirable.
Skills & Abilities
· Excellent communication and listening skills, with the ability to offer emotional and spiritual support.
· Strong organizational and documentation skills to track and manage cases effectively.
· Ability to work collaboratively across departments and with external partners.
· Proficiency in Microsoft Office, database systems, and case management software.
· Sensitivity to confidentiality and ethical considerations in ministry and social services.
Compensation and Benefits:
- Annual Salary Range: $45k to $50k Annually
- Compensation commensurate with experience and qualifications.

Working Conditions & Expectations:
· This role requires flexibility, as some situations may demand after-hours response.
· The position is on-site at New Birth Missionary Baptist Church, with occasional field visits as necessary.
