Based on Experience:
*Range for 2 year degree $27.29-$43.40
*Range for 4 year degree $28.30-$44.97
Essential Functions:
- Builds relationships with patients and coordination with primary care team, specialty care teams, and inpatient facilities.
- Develops care plans for patients with Program.
- Monitors HIT tools and reports to identify gaps in care and needed services for patients within program.
- Addresses medication alerts, hospital admissions and discharges, including emergency visits.
- Monitor and report performance measures and outcomes.
Skills Abilities:
- Ability to communicate clearly and effectively with co-workers and patients
- Ability to complete competency CPR and First Aid
- Ability to prioritize and compete multiple tasks simultaneously.
- Effective oral and written communication skills.
Working Conditions:
- Possible exposure to inside environmental conditions.
- Exposure to distressed patients, families, or visitors.
Physical Demands:
- Requires standing, sitting or walking for up to 8 hours or longer per day.
- The ability to lift up to 50 lbs.
Qualifications:
- Active Missouri RN license
Preferred Qualifications:
- Clinical experience preferred but not required
- PCHH and/or Chronic Care Management nurse experience
