Based on Experience:
Range for 2 year degree $27.29-$43.40
Range for 4 year degree $28.30-$44.97
Essential Functions:
Builds relationships with patients and coordination with primary care team, specialty care teams, and inpatient facilities.
Develops care plans for patients with Program.
Monitors HIT tools and reports to identify gaps in care and needed services for patients within program.
Addresses medication alerts, hospital admissions and discharges, including emergency visits.
Monitor and report performance measures and outcomes.
Skills Abilities:
Ability to communicate clearly and effectively with co-workers and patients
Ability to complete competency CPR and First Aid
Ability to prioritize and compete multiple tasks simultaneously.
Effective oral and written communication skills.
Working Conditions:
Possible exposure to inside environmental conditions.
Exposure to distressed patients, families, or visitors.
Physical Demands:
Requires standing, sitting or walking for up to 8 hours or longer per day.
The ability to lift up to 50 lbs.
Qualifications:
Active Missouri RN license
Preferred Qualifications:
Clinical experience preferred but not required
PCHH and/or Chronic Care Management nurse experience
