Position: Care Management Coordinator
Location: Las Vegas, NV – Onsite (This role is not eligible for Remote or Hybrid)
Starting Salary Range: $90,000 per year
The starting salary range for this role is a reasonable estimate, with a goal to pay a competitive market salary focusing near the median of our pay ranges. However, final offers for all positions will be based on several factors such as experience level, education and certification, skills, and internal pay equity.

With responsibility for assisting in identifying and facilitating appropriate transitions of care for members, the Care Management Coordinator utilizes clinical expertise to communicate care recommendations verbally and in writing to internal and external stakeholders, ensuring the these recommendations meet appropriate standards of care. The Care Management Coordinator will assist with streamling operations and improving patient outcomes throughout the patient’s course of treatment.

The Care Management Coordinator will assist with streamling operations and improving patient outcomes throughout the patient’s course of treatment.
What you’ll do:
- Collaborate with Physicians, treatment team staff, embedded care coordinators and external providers to ensure smooth transition of care.
- Identify patient needs through collaborative case conceptualization, and provide appropriate clinical input regarding treatment recommendations necessary for progress toward treatment objectives
- Collaborate with internal departments and external stakeholders to identify comprehensive services that meet patient needs
- Review patient utilization of healthcare services using clinical guidelines (e.g., InterQual, MCG, LOCUS, and ASAM) and provide recommendations for levels of care based on these reviews
- Maintain up to date knowledge of Behavioral health trends, best practices, and relevant guidelines and ensure compliance with Medicare and Medicaid insurer guidelines.
- Foster effective communication and collaboration between clinical operations and other departments to ensure seamless provision of care.
- Participate in clinician oncall rotation, providing clinical consultation and support to NBH staff and members.
- Identify areas for enhancement and implementing best practices.

- Indepth knowledge of principles and best practices in the behavioral health industry.
- Excellent understanding of regulatory and accreditation requirements.
- Strong interpersonal and communication skills for effective collaboration.
- Ability to make informed decisions and solve complex problems.
- Detailoriented and wellorganized with the ability to manage multiple tasks simultaneously.
- Exceptional critical thinking and decisionmaking abilities.
- Knowledge of electronic health record (EHR) systems.
- A minimum of 3 years of experience in clinical operations within the Behavioral health industry.
- A Master’s degree in one of the Behavioral Health Sciences.
- Must pass drug screening and background check in order to start employment with Novum Health.
- Must be willing to receive vaccinations in accordance with hospital and state requirements.
- Experience working with diverse populations in the behavioral healthcare environment.
