Case Manager

Neulife Rehabilitation is currently seeking a skilled and compassionate Case Manager to join our team in Mt Dora, Florida. As a Case Manager, you will play a vital role in providing support and guidance to individuals in need of rehabilitation services.

Responsibilities

• Conduct assessments and evaluations to determine individual needs
• Develop and implement individualized treatment plans
• Coordinate and monitor the delivery of services to clients
• Collaborate with healthcare professionals and service providers
• Advocate for clients' needs and rights
• Maintain accurate and up-to-date documentation
  
  

Requirements

• Bachelor's degree in social work, psychology, or related field
• Previous experience in case management or a related role
• Strong interpersonal and communication skills
• Knowledge of community resources and support services
• Ability to multitask and work in a fast-paced environment
