What are the responsibilities and job description for the Senior Medical Biller & Revenue Audit Lead - Chicago (Hybrid, Part-Time, 1099) position at neobill?
neobill is a new medical billing and revenue optimization firm in Chicago focused on independent practices (1 to 10 providers), with a focus on mental/behavioral health, chiropractic, orthopedic, and surgical specialties. I'm hiring our first RCM team member: a senior biller who can also help us run our revenue audits.
This is a 1099 contract role. Hours are pipeline-dependent. Some weeks will be 5 to 15 hours, other weeks heavier depending on live client work and the audit pipeline. As a brand-new firm, hours will grow steadily as we sign clients, with a clear path to full-time employee #1 of the billing team.
Chicago hybrid: most work is remote from your home office. You'll join me on occasional on-site audits at prospect practices around Chicagoland and for in-person working sessions as we build things out.
What you'd own
- Daily billing for our first live clients: claim submission, denials, appeals, A/R follow-up, payment posting, charge-entry QA
- Building our billing function from scratch: clearinghouse setup, payer enrollment workflow, SOPs, denial and A/R playbooks, QA checks. You're not inheriting a system; you're designing it.
- Documenting everything you build so the next biller can follow it
- Training new team members as we grow
- Running our Operational Revenue Audit on prospect practices (on-site and remote): undercoding review, E/M leveling, modifier and bundling analysis, workflow and A/R gaps. This is one of our core sales tools and converts prospects into clients.
- Designing our written client deliverables on the billing side. You define what each report looks like (audit reports, client A/R summaries, denial memos, QA reports), build repeatable templates, and hold the quality bar. These reports are often the first professional document a practice owner sees from us, so the bar is high.
What I'm looking for
- 7 years hands-on medical billing across independent practice, small billing firm, or RCM team
- Mental/behavioral health billing experience required. Comfortable with psych codes (90791, 90834, 90837, 90853, 96130-series), behavioral health carve-outs (UBH/Optum, Magellan, Beacon), time-based documentation, and telehealth billing rules.
- Experience setting up billing operations from scratch, not just inheriting existing ones
- Billing audit experience. You've reviewed other practices' setups, not only run your own.
- Strong documentation and training instincts. You can write an SOP another biller can actually follow.
- Professional writing. You can produce client-facing deliverables (audit reports, A/R summaries, denial memos) that read credibly to a physician owner without heavy editing.
- Comfort with startup ambiguity: you'll own and define workflows, make judgment calls without perfect instructions, and shape how this function runs.
- Chicago-based and available for occasional on-site audits and in-person working sessions.
Nice to have
- Chiropractic, orthopedic, or surgical billing experience (our pipeline skews here)
- Coding experience (CPC or equivalent), especially if you can level E/M visits from documentation
- CPB, CPMA, or similar certifications (CPMA is a real plus because it maps to our audit work)
- Fluency with major clearinghouses (Availity, Waystar, Change Healthcare) and at least two PM/EHR systems
Compensation & engagement
- $30–40/hr 1099 depending on experience and certifications
- Per-audit bonus when an Operational Revenue Audit you run converts to a signed neobill client
- Path to full-time employee #1 of the billing team as we scale
- Direct reporting to me, the founder.
Logistics
- 1099 contractor. You'll need your own workstation and a secure home office setup.
- HIPAA training and a signed BAA are required before you touch a live claim.
- Background check required.
- Non-exclusive. This is part-time, so existing consulting or 1099 engagements are fine as long as there's no conflict.
