If this sounds like an organization you would like to be a part of, we would love to meet you.The HIV and PrEP Care Coordinator will coordinate patient access to PrEP medication, HIV testing, education, linkage to care treatment, and counseling resources. This role will work in partnership with community-based organizations, patient assistance programs, behavioral health, and primary care providers to support patient care.Schedule: M-F 8am/8:30am-5pm(30 minutes or 1 hour lunch). This is a hybrid position(24 hours onsite and 16 hours remote). ResponsibilitiesConducts outreach and motivational interviewing to pre-screen and enroll patients at risk for HIV in PrEP medication and adherence programsCoordinates referrals and access to PrEP program visits, medication adherence education, and other resources such as counseling and testing services, including home rapid testsSupports outreach efforts to increase patient awareness and participation in routine HIV testing and PrEP program enrollmentsConducts HIV test education and assists with test result counseling to support linkage to HIV care treatmentConnects newly HIV and Hepatitis C diagnosed patients with health education, access to follow up visits, and support services,Assists with collaborative support resources in partnership with community partners, local government programs, and Neighborhood servicesIdentifies patient needs and refers to available risk reduction programsAssists in data collection to evaluate and improve outcomes for outreach, education, and PrEP medication adherence effortsParticipates in analysis of programs and assist with operational improvement effortsAttends and participates in required meetings, trainings, and offsite community eventsQualifications Education/ExperienceHigh school diploma/GED required; associate’s degree preferredThree years of experience in a healthcare-related field requiredExperience working within LGBTQA and other HIV high-risk communities preferredHIV Test Counselor and HIV prevention certifications and trainings preferredMedical Assistant certification preferredExperience in eCW or electronic medical records preferredBilingual (English/Spanish) highly preferredValid CA driver’s license and proof of car insurance requiredAdditional Qualifications (Knowledge, Skills and Abilities)Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with educating others about HIV, PEP, and PrEPAbility to empathize and sympathize with a high level of sensitivity to multi-cultural communitiesAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 25 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
