SCOPE OF RESPONSIBILITY:The LPN is responsible for assisting and supporting the individuals in carrying out their daily routines, their daily schedules of activities and their individual programs. The LPN is also responsible for implementing all specific clinical, educational, and vocational programs. Additional responsibilities include, but are not limited to data collection, reading and writing in daily logs, participation in program design and completion of designated areas of responsibilities. The LPN will assist Residential/CBDS participant with any health care needs and issues related to health care treatment.ESSENTIAL RESPONSIBILITIES:Implementation of clinical, educational and vocational program on a daily basis, as indicated and/or scheduledAssists in carrying out tasks and/or daily routines. Follows the established daily schedules of activitiesObserves and records accurate and reliable data for programs as established in the educational plan for the individualReads the daily log for an update on the current and procedural and programmatic information. Signs off upon completion. Makes appropriate entries in the log(s), as neededParticipates with other team members in performing all tasks necessary to maintain a clean and organized environment. Follows established cleaning schedules in the particular location. Reports or records repair needs when necessaryAttends and participates in team meetings and other required program planning meetings.Maintains positive and accurate communication throughout all work responsibilities. Works as an active and constructive team member in all dutiesAttends, completes and maintains all required trainings and certifications including MAP certificationObserves and safeguards individual's human rightsAssumes other specific responsibilities as deemed necessary to ensure the safety of the individualsMaintains a working knowledge of all individuals and will periodically work in other program locations at the request of a managerFollows the appropriate chain of command, reporting all necessary information to supervisorWhen certified in Medication Administration, dispenses medications when necessary. Follows established systems to insure quality health care of the individualsWill provide appropriate transportation when scheduled or requested.Health Care:Oversee all health care needs of Individual requiring LPN level staff.Administer medication as prescribed by MD.Participate in the continuing interdisciplinary evaluation and review of program participant.Regularly review program participant’s needs and status.Obtain reports and approvals for treatment from primary physicians.Document and communicate all recommendations from health care providers for the periodic review.Advise MDs and staff of any medical conditions that can interfere with continueddevelopment of any individual.Must be able to physically ensure the safety of all individuals in the program by safely participating in established behavioral interventionsMust maintain all certifications, and licenses as requiredQUALIFICATIONS:An active valid LPN license required.Minimum of one-year direct experience working in the fieldMaturity, good judgment and the ability to work comfortably with people in the communityExpressed interest in working with a special needs population in a behavioral settingGood communication skills, both written and verbalCurrent CPR, First Aid, Safety Care and MAP certification or ability to obtain the certificationsMust have a valid driver's licenseAbility to implement behavior plans and ISP goalsMacIntosh or PC Literacy
