Work Schedule: Monday - Friday, this is a salaried position.

Responsible for the overall clinical quality, integrity, service excellence and financial viability of Dunklau Gardens Rehab Services to ensure facility expectations are met. Plans develops, implements and monitors appropriate provision for documentation, submission for billing of clinical services and resident and family relations. Responsible for evaluating a wide variety of resident types and identifying functional capabilities and physical dysfunction to facilitate appropriated plan of care and treatment for Dunklau Gardens residents.

Essential Functions

Promotes positive relations when interacting with residents, visitors, families, physicians, customers and co-workers.

  • Greets, smiles, and makes eye contact.

  • Addresses patients and others by surname.
  • Responds quickly to patients and/or other peoples' needs.

Assist in development of operating and annual capital expense budget for the Dunklau Rehab department by completing and reviewing annual and monthly budgets, productivity reports, and prepare justification for variances in order to assure accurate fiscal management of the clinic.

  • Department strategy planning must be reflected in annual budget.
  • Ensures that productivity meets departmental goals and initiates action to meet those goals.
  • Remain within budget or provide justification for variance.
  • Be prepared to verbally defend variances as well as provide written reports when required
  • Ensure that all supplies are available.

Works with the Dunklau Gardens Administration to properly set business and marketing goals for the department and initiates an action plan to accomplish these goals to assist with growth of the clinic business.

  • By completing the business and marketing goals and plans with be completed.
  • Changes in the goals or plan will be specific and growth oriented and presented.
  • Ensures opportunities for staff participation in this process.

Develops and implements a unit-based continuous quality improvement program in order to ensure that all policies and

procedures are up to date and meet all state and facility regulatory guidelines.

  • Maintain operating procedures, rules and regulations, to ensure staff compliance with established policies and procedures continuously.
  • By reviewing quality on an ongoing basis according to policy and procedure.
  • Review and adjust policy and procedures to develop and enforce operating procedures that assures consistent quality of care.
  • Monitors department census in order to maximize therapist/clinic productivity and ensure appropriate coverage.

Conduct performance appraisals to ensure department is running smoothly and facilitate employee development.

  • By completing appraisals on an annual basis and 6 months after date of hire.
  • Completion within 30 days of the due date.
  • Information on the appraisal shall comply with the hospital guidelines on job based, performance criteria information only.
  • Specific examples and documentation should be available to support any decisions.

Properly selects employees, authorizes time cards, manages other employment issues, to ensure proper staffing levels of department operations.

  • Does so by following facility guidelines regarding interviewing, counseling and other human resource issues.

Establishes a regular means of communication with the Administrator to ensure consistent communication and

facilitate problem solving.

  • Provides a minimum of monthly meetings with Administrator required.
  • Immediate communication required for emergency situations.
  • Use e-mail, fax and phone for more frequent transmission of information.
  • Communicates noteworthy information to staff to maximize safety for all staff and patients.

Attends and participates in department planning and staff meetings and supervisors meetings and in-services to ensure

dissemination of departmental information and departmental team building.

  • By providing mandatory attendance.
  • Absences must be approved by Dunklau Gardens Administrator.

Assist with program development, including general departmental programs when indicated, to assist with the development of marketing strategies.

  • Oversees daily operations of Dunklau Gardens Rehab department.
  • Attends meetings and assisting with development and implementation of programs to achieve goals for depatment and marketing of these programs to physicians, the nursing staff and the public.
  • Develops MD relationships.

Provides in-service programs for staff, as well as participation in new employee orientation, to facilitate an active learning environment.

  • Presents a staff in-service program within one month of attendance at a professional workshop or seminar.
  • When assigned, participates in mandatory in-service and/or new employee orientation.

Evaluate a wide variety of resident types and identify physical dysfunction and functional capabilities to facilitate appropriate

treatment of resident.

  • Review medical records to assist in the evaluation process and contacts attending MD's to gather information as needed.
  • Performs an evaluation on initial visit of residents.
  • Chooses appropriate tests and measures for each diagnosis.
  • Uses appropriate tests and measures for each diagnosis.
  • Establishes a treatment plan from initial evaluation.
  • Includes resident and families in goal planning.
  • Assesses needs for additional interdisciplinary services and refers residents for these services.
  • Performs re-evaluations regularly and objectively.

Establish and implement a treatment program with regular follow-up to assure efficiency of treatment, by basing treatment on objective evaluations and functional and achievable short and long term goals with realistic time frames.

  • Treatments must be appropriate for the resident and the effectiveness of the plan is constantly being re-evaluated.
  • Frequency of treatment is appropriate for resident condition.

Provides resident focused treatments aiming to achieve maximum functional improvement with efficiency.

  • Treatment plans are appropriate to diagnosis.
  • Treatments are progressed appropriately for resident condition/status.
  • Ongoing re-assessment is evident in documentation and is addressed by modifying the plan of care.
  • Coordinates treatments with interdisciplinary services as needed.

Provide resident education to facilitate resident rehabilitation.

  • Educates each resident and/or family member regarding the status and progress of treatment
  • Instructs resident and/or family member in appropriate exercise programs, use of devise, or transfer/lifting techniques.
  • Utilizes interpreters, etc. to assist with instruction when needed/requested.

Completion of resident documentation, including initial evaluation, progress notes, discharge summary, and patient progress

reports to physicians in a timely fashion, to ensure completion of the Medical Record and enhance communication.

  • All documentation is completed within time frame according to policy.
  • Documentation includes all requirements.
  • Documentation accurately reflects services provided, resident response, and progress.
  • Documentation format and content are consistent with departmental guides.
  • Participates in clinical competencies in collaboration with MFHy Therapy department.

Resident documentation is appropriate and accurately reflects the services provided for each therapy session.

  • Indicates start/stop time of treatment, indicates time spent performing all time based codes (direct contact time), and accurately documents from this documentation.
  • Performs quarterly chart audits in Dunklau residents on case load.

Provides patient scheduling to ensure accurate scheduling of services and facilitate department operations.

  • Scheduling of residents at the end of each day for the next day.
  • May also assign residents for the therapist, assistant, and/or additional therapists.

Responsible for developing rehab tech work schedules for Rehab program.

  • Takes accountability to cover all assigned shifts.
  • Accepts responsibility for actions and consequences.
  • Facilitates time off requests for clinical staffing.
  • Fields sick calls and develops daily staffing plan.

  • Collaborates with Dunklau Gardens Administrator on updates to policies and procedures.

Participates in clinical instruction of students to facilitate their education and promote the profession and department.

  • Provides supervision and guidance appropriate for each student to ensure appropriate evaluation and treatment of patients as well as educational outcomes of the student.
  • Provides assistance when not directly supervising a student.

Job Requirements

Education

  • Graduate from Physical Therapy, Occupational Therapy or Speech-Language Pathology Program required.

Experience

  • 3 years clinical background required.
  • Prior supervisory experience or progressively responsible leadership experience required.

License/Certifications

  • Must possess and maintain current license in Nebraska required.
    • Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP) preferred.
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
Skills/Knowledge/Abilities

  • Knowledge of PT, OT or SLP therapy theory as demonstrated by successful completion of accredited physical therapy program.
  • Skill in communication, both verbally and in writing, skill in resident evaluation, application of treatment, time management and organization.
  • Skills in personnel management, problem identification and problem solving and conflict resolution.
  • Ability to work independently, adapt to a changing environment, organize and work with a variable resident load.
  • Ability to conduct research and carry though with ideas on new programs and services.
  • Ability to communicate with and interact with the public, physicians and other staff when required.
  • Ability to conduct patient teaching and in-service presentations, manage and supervise a department including facilities planning and supervision of personnel.
  • Skill in promoting positive relations and interacting with residents, visitors, families, physicians, and co-workers.
