Social Worker Opportunity:

The Social Worker (BSW) provides direct psychosocial support, case management, discharge planning, and care coordination services to adolescents and adults receiving inpatient or crisis stabilization services. This role focuses on case management, supportive therapeutic intervention, discharge planning, and collaboration with families, legal entities, and community partners to promote successful transitions from inpatient or crisis settings to appropriate levels of care.

 

What You’ll Get:

  • The starting pay range for this role is $29.84 - $32.50, based on education and experience.
  • There is 1 opening at the Youth Behavioral Health Hospital and 1 opening at the Adult Behavioral Health Hospital. 
  • This job is a full-time, in person position.

  • As a full-time employee of NCHC, you are eligible for the Wisconsin Retirement System (WRS). The WRS is a hybrid defined benefit plan. What You Bring:

 

Required:

  • Bachelor’s degree in Social Work (BSW) from an accredited institution.
  • Licensed as a Social Worker in the state of Wisconsin.
  • Valid Wisconsin driver’s license and use of a personal car.
  • More than one year of experience in behavioral health, crisis, discharge planning or case management.

What a Social Worker (in-patient) does at NCHC:

 

  • Complete comprehensive psychosocial assessments, safety plans, crisis plans, and discharge summaries.
  • Assess for risk of harm to self or others and evaluate co-occurring mental health and substance use disorders.
  • Develop and implement measurable, clinically appropriate treatment goals in collaboration with the patient and interdisciplinary team.
  • Provide individual, family, and group supportive counseling and psychoeducation consistent with licensure and scope of practice.
  • Assist with safety planning and crisis stabilization interventions.
  • Monitor patient progress and communicate updates to the treatment team.
  • Participate in daily rounds, staffing meetings, case consultations, and interdisciplinary treatment meetings to coordinate care.
  • Obtain and integrate collateral information from families, guardians, community providers, outpatient teams, and other involved supports.
  • Develop and implement individualized discharge plans based on clinical assessment and patient needs, including coordinating aftercare appointments and follow-up services.
  • Make appropriate and timely referrals to community agencies, outpatient providers, and support services.
  • Facilitate communication and coordination with families, guardians, and community providers to support continuity of care.
  • Provide clinical documentation, treatment justification, and discharge summaries to insurance providers as required.
  • Serve as a patient advocate to support safe, ethical, and person-centered care decisions.
  • Collaborate with psychiatry, nursing, behavioral health professionals, case management, CPS/APS, court liaison, and community agencies.
  • Fulfill mandated reporting responsibilities in accordance with Wisconsin statutes and professional licensure requirements.
  • Accurately document all assessments, clinical contacts, treatment updates, referrals, discharge planning, and care coordination activities in the electronic medical record.
  • Adhere to DHS, Joint Commission, HIPAA, and professional social work practice standards.
  • Ensure services are delivered in a respectful manner that upholds patient dignity, rights, trauma-informed care, and person-centered care principles.
  • Support a collaborative, team-based approach to care and contribute clinical expertise to treatment planning and team decision-making.
  • Perform other related duties as assigned.

 

Working Environment and Physical Requirements

 

  • Frequent sitting, standing, walking, bending, and reaching
  • Regular use of computer and electronic documentation systems
  • Specific vision and hearing capabilities sufficient to effectively assess patient presentation, respond to alarms or verbal cues, and maintain safety within a behavioral health environment
  • Exposure to individuals who may be agitated, hostile, or behaviorally disruptive
  • Community travel as needed for continuity of care.
  • Possible exposure to communicable diseases and bodily fluids in an in-patient setting.
