Nayar Health Care is currently seeking a PRN Home Health Physical Therapist to join our growing team.

This PRN opportunity is ideal for a Physical Therapist seeking schedule flexibility, meaningful patient care, and the option to supplement income while working with a respected home health organization.

What You’ll Do

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide skilled PT services to restore function, reduce pain, and prevent disability
• Review patient medical histories and perform assessments and reassessments
• Collaborate with interdisciplinary care teams
• Complete timely and accurate documentation in compliance with regulations
• Develop effective discharge plans to support patient independence

Requirements

• Current Physical Therapist license
• Valid driver’s license and ability to travel to patient homes
• Ability to attend occasional meetings or pick up supplies at the corporate office as needed

Preferred Experience & Qualifications

• Graduate of an accredited Physical Therapy program
• Current CPR certification
• Minimum one year of experience as a Physical Therapist
• Home health or hospice experience preferred
