Job Title: Registered Nurse (RN) – Long-Term Care
Location: Berlin, New Hampshire
Shift: Day Shift
Schedule: 12-hour shifts
Guaranteed Hours: 36 hours per week
Job Summary
"Navitas Healthcare, LLC" is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality patient care in a long-term care and rehabilitation setting. The RN will be responsible for monitoring patient health, administering treatments, coordinating care plans, and ensuring a safe and supportive environment for residents.
Key Responsibilities
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor residents’ conditions, assess changes, and report updates to the healthcare team
- Develop, implement, and update individualized patient care plans
- Supervise and collaborate with LPNs, CNAs, and other support staff
- Maintain accurate and timely patient documentation
- Ensure compliance with clinical standards, safety policies, and regulatory requirements
- Provide compassionate support and education to residents and their families
Qualifications
- Active Registered Nurse (RN) license in New Hampshire
- Previous Long-Term Care (LTC) or rehabilitation experience preferred
- Strong clinical assessment and documentation skills
- Ability to work effectively in a collaborative healthcare environment
