Position Title: Registered Nurse (RN) – Case Management
Location: Hyannis, MA 02601
Job Type: Contract (13 Weeks)
Schedule:
- Flexible shifts – 4 days/week (10–10.5 hours per shift)
- Monday–Friday with rotating days, weekends, and holidays
- Potential evening/night coverage: 11:30 AM – 10:00 PM or 12:00 PM – 10:30 PM
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced Case Management Registered Nurse (RN) to coordinate and facilitate patient care within an acute care setting. This role focuses on delivering high-quality, efficient, and cost-effective care while collaborating with interdisciplinary teams. The ideal candidate will have strong expertise in discharge planning, utilization review, and patient advocacy.
Key Responsibilities
- Screen new admissions daily to identify discharge planning needs and prioritize patient visits
- Verify patient demographics and insurance information; communicate updates as needed
- Conduct initial and ongoing assessments using InterQual® or equivalent criteria
- Review appropriateness of admissions and level of care
- Initiate required notifications, including HINN notices and appeal processes
- Identify high-risk patients and coordinate appropriate referrals
- Facilitate utilization review and support third-party reimbursement processes
- Participate in interdisciplinary rounds and care conferences
- Communicate effectively with patients and families regarding care plans and insurance coverage
- Maintain accurate documentation and ensure compliance with healthcare regulations
- Collaborate with care teams to ensure seamless patient transitions and discharge planning
Required Qualifications
- Minimum 3 years of acute care experience within the past 5 years
- Recent experience (within 4 years) in discharge planning and utilization review
- Working knowledge of InterQual® or similar utilization review systems
- Strong organizational, communication, and negotiation skills
- Ability to work independently in a fast-paced environment
- Proficiency with healthcare systems and computer applications
Education
- Preferred: Bachelor of Science in Nursing (BSN)
- Case Management certification (CPUM or specialty certification) preferred
Licensure & Certifications
- Active Massachusetts RN License (required)
- Acute care experience (required)
- InterQual® knowledge (required)
