Job Title: Registered Nurse (RN) – Case Manager
Location: Apple Valley, CA 92307
Duration: 6 Weeks
Schedule: Day Shift (5x8) – 8:00 AM to 4:30 PM
Hours: 40 Hours per Week
Compensation
- Weekly Pay: $2,300 – $2,600
Position Overview
A leading healthcare facility is seeking an experienced Registered Nurse (RN) – Case Manager to provide acute care case management and discharge planning services within a hospital setting. The ideal candidate will coordinate patient care, optimize resource utilization, and ensure safe and efficient patient discharges.
Key Responsibilities
- Manage acute care case management and discharge planning activities.
- Develop and coordinate patient care plans.
- Collaborate with interdisciplinary teams, patients, and families.
- Ensure appropriate utilization of healthcare resources.
- Facilitate safe, timely, and effective patient discharges.
- Maintain accurate documentation and comply with healthcare standards.
Required Qualifications
Education:
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Certifications & Licensure:
- Active California RN License (pending accepted; must be active before start)
- BLS (AHA)
Required Experience
- Minimum 2 years of Acute Care Case Management / Discharge Planning experience
- Recent hospital experience required (behavioral health experience not accepted)
- Must be a seasoned traveler (first-time travelers will not be considered)
Additional Requirements
- Weekend coverage required (every other weekend)
- Patient ratios:
- Days: 1:25
- Weekends: 1:45
- Maximum 7 days RTO (Requested Time Off)
