Position Overview:
"Navitas Healthcare, LLC" is seeking a licensed Physical Therapist to provide in-home therapy services in Hood River, OR, and surrounding areas. This role is ideal for skilled PTs interested in patient-focused, independent home health care with flexible scheduling.
Key Responsibilities:
Provide skilled PT services in patients’ homes (up to 6 visits per day).
Develop and implement individualized care plans to improve functional independence.
Conduct patient assessments, monitor progress, and adjust therapy interventions as needed.
Document all patient visits using EPIC/Oasis software.
Ensure patient safety and adherence to home health regulations.
Coordinate care with nurses, physicians, and other multidisciplinary team members.
Requirements:
Active Oregon PT license.
1 year of skilled home health experience required; first-timers accepted.
BCLS certification.
Reliable transportation for home visits.
Oasis and EPIC experience preferred.
Schedule:
Monday – Friday, 08:00 AM – 04:30 PM
5x8-Hour shift
No weekends required
No block schedule accommodation
Logistics & Benefits:
