Position: LPN – Long-Term Care (LTC)
Location: Rochester, NY
Duration: 8 Weeks (Contract)
Schedule: Evening/Night Shift | 5x8 Hours
Guaranteed Hours: 40 hours/week
Compensation:
- Pay Rate: $32 – $34/hour ($1,280 – $1,360/week)
- Note: Local candidates are not accepted (must reside more than 75 miles from facility)
Job Summary:
We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care in a long-term care and rehabilitation setting. The ideal candidate will support patient care, monitor conditions, and ensure compliance with care plans in a fast-paced skilled nursing environment.
Key Responsibilities:
- Deliver nursing care to residents in LTC and rehab units.
- Monitor patient conditions and report changes to the care team.
- Perform blood glucose monitoring and related testing.
- Administer medications and treatments as prescribed.
- Maintain accurate and timely patient documentation.
- Float between multiple units within the facility as required.
- Ensure adherence to safety, infection control, and facility protocols.
Patient Population:
- Mix of Long-Term Care (LTC) and Rehabilitation patients
Required Qualifications:
Experience:
- Experience in Skilled Nursing Facility (SNF) or Nursing Home setting required
Certifications:
- BLS (Basic Life Support) – AHA required
Skills:
- Blood glucose monitoring and testing
- Strong patient care and clinical observation skills
Additional Requirements:
- Must be willing to float across all 3 units within the facility
- Weekend Requirement: Every other weekend (total of 8 weekend shifts during contract)
- Candidates must reside more than 75 miles from the facility
