Position Overview
"Navitas Healthcare, LLC" is sseking for an experienced Licensed Practical Nurse (LPN) to provide comprehensive patient care in a Long Term Care setting. The role includes medication administration, patient monitoring, and supporting residents with complex medical needs.
Key Responsibilities
- Perform patient assessments and provide prescribed treatments
- Administer medications and monitor patient responses
- Report significant changes in patient condition to MD/NP
- Assist with admissions and discharges
- Support activities of daily living (ADLs)
- Provide specialized care, including:
- Gastrostomy tube care
- Catheter care
- CPAP support
- Nebulizer treatments
- Wound vac management
- Maintain accurate documentation in Meditech EMR
- Ensure compliance with care standards and safety protocols
Required Qualifications
- Active LPN license (required upon acceptance)
- Minimum 2 years of Long Term Care (LTC) experience
- Current BLS/BCLS certification
- Flu vaccination or approved exemption required
Preferred Qualifications
- Experience with Meditech EMR
