What You’ll Do

• Perform comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care including wound care, medication management, and disease education
• Collaborate closely with physicians, therapists, and caregivers
• Monitor patient progress and intervene early to prevent hospitalizations
• Educate patients and families to promote independence and safety at home
• Maintain accurate, timely documentation

✅ What We’re Looking For

• Active RN license in Virginia
• Home health experience preferred (but we will train strong candidates!)
• Strong clinical judgment and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and manage time effectively
• Reliable transportation
