What You’ll Do
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care including wound care, medication management, and disease education
- Collaborate closely with physicians, therapists, and caregivers
- Monitor patient progress and intervene early to prevent hospitalizations
- Educate patients and families to promote independence and safety at home
- Maintain accurate, timely documentation
✅ What We’re Looking For
- Active RN license in Virginia
- Home health experience preferred (but we will train strong candidates!)
- Strong clinical judgment and critical thinking skills
- Excellent communication and patient education abilities
- Ability to work independently and manage time effectively
- Reliable transportation
