Diabetes Educator

Job Summary
We are seeking a dedicated and knowledgeable Diabetes Educator to join our healthcare team. The ideal candidate will possess a strong background in patient education, nutrition, and chronic disease management, with a focus on diabetes care. As a Diabetes Educator, you will empower patients to manage their condition effectively through personalized education, comprehensive care plans, and ongoing support. Responsibilities

• Provide comprehensive diabetes education to patients and their families, including management of blood glucose levels, medication adherence, and lifestyle modifications.
• Develop individualized care plans based on thorough patient assessments, diagnostic evaluations, and physiological considerations.
• Educate patients on nutrition principles, including carbohydrate counting, meal planning, and weight management strategies.
• Supervise and collaborate with multidisciplinary teams to ensure cohesive patient care, especially in hospital or long-term care environments.
• Utilize electronic medical record (EMR) systems to document patient interactions, progress notes, and care plans accurately.
• Conduct training sessions on specialized topics such as tube feeding techniques and insulin administration.
• Monitor patient progress over time, adjusting care strategies to optimize health outcomes related to diabetes and geriatrics populations.
• Stay current with advances in diabetes management, including new therapies and diagnostic tools, to provide evidence-based guidance.

Pay: $45.00 - $50.00 per hour
